This is a brief, evidence-based screening tool developed in partnership with the World Health Organization. It explores traits related to attention, focus, impulsivity, and task management.
You might recognize some of these traits if you’ve ever struggled with mental clutter, forgetfulness, time blindness, or difficulty finishing things, even things you care about.
This form is not a diagnosis, but it helps identify patterns that may support a referral for further evaluation.

The AQ is a self-assessment designed to explore traits commonly associated with autism in adults. It touches on how you process social interaction, routine, sensory input, and imaginative thinking.
Many people find it helpful when trying to understand lifelong traits they’ve masked, adapted around, or simply never had a name for.
This tool does not diagnose autism but may help clarify whether further evaluation could be helpful.

This tool looks at patterns of repetitive thoughts and behaviors, such as checking, organizing, mental loops, or needing things “just right.”
It’s especially useful for identifying OCD-related traits that can overlap with autism or anxiety and may go unrecognized if they’re internal (like mental reviewing or reassurance-seeking).
This screening can support a more accurate referral if obsessive traits are impacting daily function.

These following forms are here only if you're curious and want to explore related areas like mood, sensory sensitivity, or executive function. You can skip them entirely or choose just the ones that feel relevant to your experience.
If you do choose to complete any of these optional forms, your responses will be reviewed and included in your personalized summary to give a fuller picture of how your traits may interact.
These screeners focus on anxiety, low mood, and emotional stress. Many neurodivergent people experience emotional ups and downs, sometimes due to burnout, masking, or feeling misunderstood. These forms can help clarify how your emotional state may be affecting your energy, focus, or day-to-day life.
GAD-7 – General Anxiety Screen
Screens for patterns of worry, restlessness, overthinking, or feeling on edge.
Optional if you often feel “on high alert” or anxious, even when nothing specific is wrong.

PHQ-9 – Depression Screen
Looks at low mood, fatigue, lack of motivation, or changes in sleep or appetite.
Optional if you’ve been feeling emotionally low or mentally drained.

DASS-21 – Depression, Anxiety, and Stress Scale
Combines all three emotional areas (depression, anxiety, stress) into one form.
Optional if your emotional state feels complex or hard to describe.

It’s especially useful for those who were missed in childhood, or who are late-diagnosed or self-identifying.
